We are seeking a highly skilled and compassionate Part-Time Evening LPN Care Coordinator to join our team at Astral at Franklin. As a valued member of our interdisciplinary team, you will play a critical role in delivering exceptional nursing care to our residents and promoting a holistic approach to their well-being.
">Key Responsibilities:

Lead the daily functions of certified nursing assistants and resident care associates, ensuring compliance with federal, state, and local regulations as well as company policies.
">Promote holistic care for residents following the nursing model, focusing on individualized needs and preferences.
">Assist with the admission, discharge, and transfer processes for residents when required, ensuring a smooth transition.
">Assess changes in residents' conditions, notifying physicians and families as necessary, and documenting all findings accurately.
">Transcribe and execute physician orders promptly, maintaining up-to-date medical records.
">Administer medications as prescribed, monitoring for any adverse effects and reporting concerns to the relevant authorities.
">Perform wound treatments as directed by physicians, observing for changes and documenting appropriately.
">Schedule follow-up appointments and coordinate transportation for residents as needed, ensuring timely access to care.
">Provide nursing leadership and support to assigned personnel on the unit, fostering a positive and collaborative work environment.
">Deliver direct care services, including colostomy changes, tube feeding, wound care, suctioning, and IV administration, in accordance with established protocols.
">Conduct regular rounds to ensure resident needs are met and staff duties are fulfilled, promoting a culture of excellence and accountability.
">Collaborate effectively with interdisciplinary team members to comprehensively address residents' needs, leveraging expertise and resources to achieve optimal outcomes.
">Report any incidents or unusual occurrences to community leadership, ensuring prompt investigation and resolution.
">Maintain adequate stock of medications and supplies, notifying the appropriate personnel when items are low.
">Initiate, review, and update care plans as required, ensuring resident-centered and evidence-based practice.
">Participate in survey processes during inspections, demonstrating a commitment to quality and regulatory compliance.
">Engage in QAPI activities as assigned, contributing to continuous improvement and innovation in care delivery.
">Follow strict infection control principles and help correct any deficiencies noted, ensuring a safe and healthy environment for residents and staff.
">Perform additional duties as assigned, complying with all policies and procedures promptly and efficiently.

Associate's or Bachelor's degree in Nursing from an accredited institution.
">Current unrestricted LPN license in practicing state and current CPR certification.
">Minimum of 1 year nursing experience preferred, with a proven track record of delivering high-quality care and excellent customer service.
