Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at Community Health Resources. As a LPN, you will play a vital role in providing quality care to our clients in a methadone maintenance program.

Responsibilities:
  • Perform initial nursing assessments and document ongoing assessments of patients' response to treatment.
  • Develop, implement, and evaluate nursing care plans according to age-specific needs of patients.
  • Responsible for accurate input of computer data.
  • Execute the medical and medication regimen of care as prescribed by the physician.
  • Provide clients with daily methadone doses and prescribed take-home doses.
  • Monitor urine toxicology screens and breathalyzers.
  • Administer PPD tests.
  • Educate the patient/family toward an understanding of healthy living.
  • Respond to medical/psychiatric emergency needs of patients, following CHR policies and procedures.
Qualifications:
  • Education: Degree/Certification in nursing or in a related healthcare field preferred.
  • Experience: None required.
  • Licensure/Certification/Registration: Current RN or LPN license in the State of Connecticut required, Certification in CPR, and must have a valid driver's license.
