LPN Job Description

Job Responsibilities

As an LPN with IntelyCare, your responsibilities will include:

  • Delivering post-acute care to patients
  • Monitoring the physical and mental wellbeing of patients and tracking their vital signs
  • Performing intakes of residents and documenting their health concerns and medical histories
  • Reviewing medical records and charts of residents
  • Properly administering and verifying medications
  • Helping to maintain appropriate levels of medical supplies
  • Contributing to a safe, comfortable environment for residents
  • Assisting with IV set up, vaccinations, and phlebotomies
  • Helping medical staff with orientation and training
Qualifications and Skills

To be eligible for this LPN position, you must:

  • Hold a current Connecticut state license to practice as a Licensed Practical Nurse (LPN)
  • Have completed an approved Licensed Practical Nurse (LPN) educational program
  • Have a high school diploma (or GED/equivalent)
  • Have at least 1 year of experience as an LPN or in a related role
