Position Overview:

Key Responsibilities:
  • Deliver personalized, one-on-one patient care with an emphasis on quality of life.
  • Work weekends, specifically Saturdays and Sundays from 8:00 am to 5:00 pm, with the possibility of evening and night shifts as needed.
  • Provide expert nursing services to hospice patients in various settings, including homes, care facilities, and hospitals.
  • Conduct initial and ongoing assessments of patients, adjusting care plans as necessary.
  • Collaborate with an interdisciplinary team to coordinate comprehensive care for patients and their families.
  • Utilize critical thinking and teaching skills to empower family caregivers during the end-of-life journey.
  • Maintain accurate and timely documentation of patient care and interventions.
  • Manage your daily schedule in accordance with agency policies and regulatory requirements.

Qualifications:
  • Minimum of one year of recent clinical experience as a Registered Nurse, with a preference for two years in hospice or home health settings.
  • Certification as a Hospice and/or Palliative Care Nurse is preferred.
  • Strong clinical nursing skills and a commitment to ongoing professional development.
  • Ability to thrive in high-pressure situations while managing multiple priorities.
  • Proficiency in computer systems, including EMR and Microsoft Office.
