Job Opportunity

Serenity Hospice is seeking a skilled Hospice RN Case Manager to join their team in Salem, OR. This role offers a competitive salary, generous time off packages, and comprehensive benefits.

Key Responsibilities:

  • Provide triage support for after-hours patient and family phone calls, determining the need for services and delegating nursing visits as needed.
  • Conduct assessments, symptom management, and care plan updates to ensure optimal patient care.
  • Communicate with the attending physician, interdisciplinary staff members, and other agencies to coordinate care.
  • Perform clinical pronouncements of death and provide support to families.
  • Maintain accurate clinical records through timely preparation of care plans and clinical notes.

Requirements:

  • Graduate from an accredited registered nursing program.
  • Current license to practice as a Registered Nurse in the state.
  • Minimum of two years of varied work experience as a professional nurse.
  • Demonstrated skill in nursing practices and clinical judgments.
  • Ability to cope with family emotional stress and tolerance of individual lifestyles.
  • Current CPR Certification.
  • Valid driver's license and proof of insurance.
