Job Overview

POSITION SUMMARY:

  • The primary responsibilities of the Licensed Practical Nurse (LPN) include, but are not limited to, the following essential functions, which may be updated periodically at the discretion of Viva Senior Living:
  • Adhere to business conduct standards in compliance with federal, state, and local health regulations and guidelines.
  • Possess a valid LPN license issued by the State of Pennsylvania.
  • Hold at least a high school diploma or GED.
  • Demonstrate the ability to collaborate effectively with colleagues in a supportive work environment while maintaining compassion and dignity for residents, in accordance with the Resident Bill of Rights.
  • Be available for emergency call-backs during critical situations (e.g., severe weather, evacuations, staffing shortages).
  • Follow Viva Senior Living's attendance policies and the terms outlined in the Employee Handbook.
  • Exhibit proficiency in reading, writing, speaking, and understanding English.
  • Uphold the highest standards in caring for residents and their families.
  • Comply with all facility policies and procedures.

RESPONSIBILITIES/ACCOUNTABILITIES:

  • The LPN is accountable for delivering care, treatment, and medication administration to assigned residents, continuously monitoring and assessing their conditions, and reporting in accordance with care plans, established medical practices, and applicable laws and regulations, as well as Viva's policies.
  • Oversee operations within the Memory Care Unit.
  • Assist in supervising ancillary care staff in the absence of the Resident Care Director.
  • Provide care to assigned residents based on individualized care plans.
  • Administer medications while adhering to the Five Rights of Medication Administration.
  • Manage the reordering and receipt of medications.
  • Deliver personal care and assistance with Activities of Daily Living (ADLs).
  • Supervise care staff effectively.
  • Maintain accurate shift-to-shift reports.
  • Offer emergency assistance to residents with appropriate follow-up actions.
  • Obtain and transcribe physician orders accurately.
  • Maintain comprehensive case management documentation.

PHYSICAL AND SENSORY REQUIREMENTS:

(With or Without Mechanical Aids)

  • Ability to move intermittently throughout the workday.
  • Capacity to manage the mental and emotional demands of the role.
  • Ability to see and hear, or utilize prosthetics that enable these senses to function adequately for fulfilling job requirements.
  • Meet general health requirements as per facility policies, including medical and physical examinations.
  • Ability to relate to and work with ill, disabled, elderly, emotionally distressed, and occasionally agitated residents.
  • Capability to push, pull, move, and/or lift a minimum of 25 pounds to a height of at least 5 feet and carry such weight a minimum distance of 50 feet.
  • May be required to assist in the evacuation of residents during emergencies.
