Position Overview

SUMMARY OF ROLE:

  • The fundamental responsibilities of the Licensed Practical Nurse (LPN) include, but are not limited to, the following:
  • Adhere to ethical standards of practice in compliance with federal, state, and local health regulations and guidelines.
  • Possession of a valid LPN license issued by the State of Pennsylvania is required.
  • A minimum of a high school diploma or GED is necessary.
  • Ability to collaborate effectively with colleagues in a supportive work environment while demonstrating compassion and respect for residents, in alignment with the Resident Bill of Rights.
  • In emergency situations, the employee may be required to return to work during critical conditions (e.g., severe weather, evacuations, staffing shortages).
  • Compliance with attendance policies and adherence to the terms outlined in the Employee Handbook is mandatory.
  • Proficiency in reading, writing, speaking, and understanding English is essential.
  • Commitment to maintaining the highest standards in meeting the needs of residents and their families.
  • Compliance with all facility policies and procedures is expected.

KEY RESPONSIBILITIES:

  • The LPN is tasked with delivering care, treatment, and medication administration to assigned residents, while continuously monitoring and evaluating their conditions in accordance with care plans, established medical practices, and relevant laws and regulations.
  • Oversight of the Memory Care Unit is a critical responsibility.
  • Assist in supervising ancillary care staff in the absence of the Resident Care Director.
  • Deliver care to residents based on individualized care plans.
  • Administer medications with strict adherence to the Five Rights of Medication Administration.
  • Manage the reordering and receipt of medications.
  • Provide personal care and assistance with activities of daily living (ADLs).
  • Supervise care staff to ensure quality service delivery.
  • Maintain accurate shift-to-shift reports.
  • Provide emergency assistance to residents with appropriate follow-up actions.
  • Obtain and accurately transcribe physician orders.
  • Maintain comprehensive case management documentation.

PHYSICAL AND SENSORY REQUIREMENTS:

  • Ability to move intermittently throughout the workday is essential.
  • Capability to manage the mental and emotional demands of the role is required.
  • Must possess adequate vision and hearing, or utilize prosthetics that ensure these senses function effectively to meet job requirements.
  • Compliance with general health standards set forth by facility policies, including medical and physical examinations, is necessary.
  • Ability to relate to and work with ill, disabled, elderly, and emotionally distressed residents is crucial.
  • Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a height of at least 5 feet and carry such weight a minimum distance of 50 feet.
  • Assistance in the evacuation of residents during emergencies may be required.
