This role involves a variety of clinical social work services within a primary care or home health environment. The successful candidate will provide support and guidance to the care team in managing patient needs. Responsibilities include evaluating psychosocial strengths and challenges to assist patients in achieving maximum independence. Collaboration with other professionals to develop comprehensive care plans is essential. The role also includes offering short-term counseling, education, and resource navigation for caregiving needs.



Key Responsibilities:
  • Deliver appropriate social work interventions to patients of all ages and their families, including psychosocial assessments, brief individual or family counseling, advocacy, referrals, group work, case coordination, and long-term care planning.
  • Engage in consultation and coordination with a multidisciplinary team of healthcare professionals, including participation in family and care conferences.
  • Assess patient and family needs for care, ensuring effective communication with other healthcare providers. Collect and analyze relevant data throughout the care process.
  • Continuously evaluate patient and family needs through timely assessments and observations, adjusting care plans as necessary based on responses.
Qualifications:Experience:
  • Minimum of one (1) year of prior social work experience.
  • Current KP Coalition employees may have this experience requirement waived.
Education:
  • Master's degree in social work (MSW).
Licensure:
  • Licensed Marriage and Family Therapist (Washington) OR Licensed Advanced Social Worker License (Washington) OR Licensed Social Worker Associate-Independent Clinical (Washington) OR Licensed Independent Clinical Social Worker (Washington) OR Mental Health Counselor License (Washington).
Additional Requirements:
  • Strong written and verbal communication skills.
  • Proven ability to work effectively within a multidisciplinary team and a commitment to diversity.
Preferred Qualifications:
  • Two (2) years of post-master's experience.
  • Experience in care coordination within managed care systems, inpatient/outpatient settings, skilled nursing, or long-term care facilities.
  • Knowledge of social case management, conflict resolution, and team-building practices.
  • Familiarity with Microsoft Office and electronic charting systems.
