RN Care Manager

Our community in Louisville, CO offers a supportive environment for residents in need of assisted living and skilled nursing facilities. As a Nurse Care Manager, you will be responsible for delivering high-quality nursing services and care to assigned patients in accordance with organizational policies and applicable regulations.
Key Responsibilities:
- Provide direct patient care, following nursing policies and procedures, and collaborate with other team members and healthcare providers as needed.
- Assess residents' physical and psychological conditions, providing comprehensive clinical nursing care, including medication administration, treatments, pain management, and symptom management.
- Report adverse reactions to medications or treatments in accordance with company policy and consult with healthcare team members and physicians to assess, plan, implement, evaluate, and update resident care plans.
- Participate in the care planning process, oversee implementation of care plans, and complete necessary charting as required.
- Communicate with physicians regarding changes in condition, diagnostic test results, and educate residents and their families on health-related issues.
Requirements:
- Current/valid RN license in the state of Colorado, including multi-state.
- CPR certification.
- Knowledge of nursing and medical practices, procedures, laws, regulations, and guidelines pertaining to long-term care.
- Competence with computer applications.
- Ability to communicate effectively verbally and in writing with residents, families, staff, and the community.
