Job Summary

Eden Health is seeking a skilled Registered Nurse to provide high-quality care to our patients in the comfort of their own homes. As a Home Health RN, you will be responsible for delivering comprehensive care, managing patient cases, and collaborating with our interdisciplinary team to ensure optimal patient outcomes.

Key Responsibilities
  • Provide admission, case management, and follow-up skilled nursing visits for Home Health patients
  • Administer ongoing care and case management, providing necessary follow-up as directed by the Clinical Manager
  • Implement patient care plans in conjunction with patients and families to achieve optimal resolution of needs and problems
  • Coordinate and oversee the work of Home Health Aides, providing written instructions that reflect current plans of care
  • Discharge patients after consultation with the physician and Clinical Manager, preparing and completing necessary clinical documentation
Requirements
  • Graduate of an accredited nursing program
  • Current licensed Registered Nurse (RN) in the state of practice
  • Experience as an RN in an acute-care setting or Home Health setting preferred
  • Credentials in specialty areas preferred (wound care, OASIS certification, IV, ET)
  • Valid driver's license and reliable transportation
  • Proof of automobile insurance (MVR checked)
  • Proof of current CPR certification
  • Able to pass criminal background check and national sex offender clearance
