We are seeking an experienced Licensed Practical Nurse to join our team at Puget Sound Home Health & Hospice. As a home health LPN, you will provide high-quality care to patients in their own homes, working collaboratively with our interdisciplinary team to deliver exceptional patient outcomes.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to meet those needs.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
  • Document patient care and progress in accordance with regulatory requirements and agency policies.
Requirements:
  • Graduate of an accredited practical nurse or vocational nursing program.
  • Three years of nursing experience preferred.
  • Currently licensed LPN or LVN in Washington.
  • CPR/BLS certification from AHA.
