Responsibilities:- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education.
- Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
- Communicate effectively with patients, families, and caregivers to ensure seamless care transitions.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Graduate of an accredited practical nurse or vocational nursing program.
- Three years of nursing experience preferred.
- Currently licensed LPN or LVN in Washington.
- CPR/BLS certification from AHA.
- A competitive hourly rate of $30.00-$36.00, depending on experience.
