About the RoleAs a Home Health Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients. You will make visits to clients in their homes, perform diagnostic tests and measurements, and develop and implement individualized care plans. Your responsibilities will include educating and instructing clients and family members on rehabilitative care and activities necessary for independent living.
Key Responsibilities- Making home visits to assess client needs and develop care plans
- Performing diagnostic tests and measurements, such as mobility assessments and strength evaluations
- Developing and implementing individualized care plans, including manual therapeutic exercises and gait training
- Educating and instructing clients and family members on rehabilitative care and activities
- A current license as a Physical Therapist in Massachusetts

The estimated salary for this role is $10,000 per month, based on industry standards and location.
