Caregiving Specialist

BAYADA Home Health Care is seeking a compassionate and dedicated Registered Nurse (RN) to provide home health visits in Fairhaven, Massachusetts.

• We are looking for a skilled RN to join our team and deliver exceptional home health care to adult and geriatric patients in their homes.
• You will be responsible for making home visits, assessing clients' needs, and developing personalized care plans to ensure their comfort, independence, and dignity.

• Follow designated care plans to meet clients' evolving needs
• Conduct thorough assessments and document observations, interventions, and evaluations using a touch pad tablet
• Develop and implement comprehensive care plans in collaboration with clients, families, and healthcare teams
• Provide education and support to clients and their caregivers on disease management, medication administration, and wound care
• Monitor clients' conditions and report changes to Clinical or Client Services Managers

• A current license as a Registered Nurse in Massachusetts
• Graduation from an accredited and approved nursing program
