Key Responsibilities:
Participate in departmental orientation and complete initial and annual competency assessments.
Collaborate with other departmental professionals to enhance patient care delivery, identify obstacles to care or discharge, and devise effective solutions.
Ensure timely and accurate documentation, including the Individual Plan of Care (IPoC) in accordance with CMS guidelines.
Organize family meetings and maintain communication with caregivers to keep them informed about patient progress and rehabilitation goals.
Facilitate weekly patient care team meetings to develop, monitor, and adjust treatment plans aimed at achieving patient objectives.
Assess the patient's assigned Case Mix Group (CMG) and assist the team in recognizing any overlooked comorbid conditions being treated during the patient's stay, communicating findings to the Health Information Management (HIM) team.
Perform additional duties as required.Qualifications:
Current licensure as a Registered Nurse, Social Worker, or other healthcare professional such as a Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist.
Preferred certifications include Case Management or Rehabilitation Nursing, such as those from the Commission for Case Manager Certification (CCM), Association of Rehabilitation Nurses (ARN), American Case Management Association (ACM), or Board Certification in Case Management by the ANCC.
Experience:
A minimum of 2 years of experience in social work or case management within an inpatient environment is highly preferred, particularly in acute or rehabilitation hospital settings.
Strong oral and written communication skills in English are essential; proficiency in additional languages is a plus.
Basic computer proficiency in applications such as Excel, Word, Outlook, and PowerPoint is required.
