As a Clinical Nurse Liaison, your responsibilities will include:

  • Educating healthcare professionals about hospice services and facilitating end-of-life solutions grounded in clinical excellence and compassion.
  • Ensuring the intake and referral processes are executed efficiently to meet the needs of patients, families, and referral sources.
  • Completing the admissions process for referred patients in urgent situations, as staffing allows, and for inpatient hospice admissions.
  • Providing accurate and timely information regarding hospice services to all inquiries.
  • Responding promptly to calls from referral sources and initiating the necessary paperwork to commence the intake/referral process.
  • Acting as a patient advocate in assessing and developing care plans and securing informed consent for hospice services.
  • Engaging with various account types, including skilled nursing facilities, assisted living facilities, hospitals, and physicians.
  • Delivering in-service training and educational sessions on the advantages of hospice care.

Qualifications:

  • A compassionate and empathetic approach, demonstrating a genuine hospice heart towards clients, their patients, and families.
  • Active RN license in the relevant state(s).
  • A minimum of two years of healthcare experience is preferred, ideally in settings such as hospice, home health, clinical liaison roles, or as a hospice case manager.
  • A strong reputation and respect within the community.
