Job Summary

South Coast Children's Society, Inc. is seeking a highly skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in the overall coordination of intensive, family-centered, community-based services. Your primary responsibility will be to facilitate Child and Family Team (CFT) Meetings and guide the evolution of the Plan of Care to promote a successful home placement for the referred youth.

Key Responsibilities
  • Child and Family Team (CFT) Coordination: Effectively manage and coordinate all aspects of the CFT, including scheduling and facilitating meetings, involving the family in the planning and decision-making process, and ensuring that services are relevant to established goals and needs.
  • Strengths-Based Assessments: Perform initial and ongoing strengths-based assessments of children and families to identify and prioritize their needs across all domains.
  • Plan of Care Development: Coordinate the development of both the Plan of Care and Safety Plan, ensuring that services are relevant to established goals and needs as expressed by the CFT.
  • Documentation and Communication: Maintain proficient and accurate documentation, adhering to all program and contract standards for compliance, productivity, and timeliness, and interface and collaborate with various stakeholders, including Orange County Social Services, Orange County Healthcare Agency, Orange County Probation, court systems, law enforcement, schools, health care providers, and other community liaisons.
Requirements
  • Education: Bachelor's Degree in social work, psychology, nursing, occupational therapy, or a related field from an accredited college or university.
  • Experience: One year of experience in human services, preferably case management, and one year of experience working with the target population.
  • Licensure and Certification: Valid CA driver's license, proof of automobile insurance, and completion of CPR/First Aid certification within the first thirty (30) days of employment.
Working Conditions

The Care Coordinator will work in a fast-paced, dynamic environment, requiring strong organizational and communication skills, as well as the ability to remain calm and composed in high-pressure situations. The position may also require occasional travel to client settings and participation in an after-hours on-call rotation.
