Key Responsibilities- Direct and coordinate the care provided by a multi-disciplinary team of home health care professionals.
- Process incoming referrals and manage internal case management.
- Lead the management and mentoring of Clinical Associates and field staff.
- Develop and implement quality assurance programs to ensure high-quality patient care.
- Monitor staff productivity and overall case management.
- An active Hawaii Registered Nurse license.
- Home Health experience preferred.
- Recent Medicare Certified Home Health Care experience.
- Comprehensive knowledge of the Medicare Conditions of Participation.
- Thorough understanding of OASIS documentation and ICD-10 coding.
- Strong people management and leadership experience highly desired.
- Excellent organizational, interpersonal, and communication skills.
- Starting salary at $95,000 based on qualifications.
- Full-time, office position with flexibility and work-life balance.
