Key Responsibilities:- Direct and coordinate care provided by a multi-disciplinary team of home health care professionals.
- Process incoming referrals and manage staff productivity.
- Develop and mentor Clinical Associates and field staff.
- Ensure accurate documentation, including OASIS and Home Care Home Base (HCHB).
- Monitor quality assurance and manage staff performance.
- Active Hawaii Registered Nurse license.
- Home Health experience preferred.
- Recent Medicare Certified Home Health Care experience.
- Comprehensive knowledge of Medicare Conditions of Participation.
- Strong people management and leadership experience highly desired.
- Starting salary at 95k based on qualifications.
- Full-time, office position with flexibility and work-life balance.
