Gentiva, a leading provider of home health and personal care services, is seeking a highly skilled Field Supervisor PC to join our team.

Job Summary

The Field Supervisor PC will be responsible for conducting on-site client evaluations, developing service plans, and performing other field visits as assigned. This role will also involve recruiting, hiring, training, and supervising attendants.

Responsibilities

  1. Conduct on-site client evaluations and develop service plans in conjunction with authorization.
  2. Perform other field visits as assigned.
  3. Recruit, hire, train, and supervise attendants.
  4. Complete the appropriate forms, attach to the Daily Visit Report, and turn into the office within 48 hours of visit.

Requirements

To be eligible for this role, candidates must have two years of full-time study at an accredited college or university or equivalent experience in a supervisory capacity in a healthcare facility, agency, or community-based agency. PHC and MC experience are preferred.
