Caspar Mountain Rehabilitation and Care Center is seeking a highly skilled and experienced MDS Coordinator to join our dynamic leadership team.
Key Responsibilities:
- Assist the Resident Care Management Director in the timely and accurate completion of the RAI and Care Management process from admission to discharge, adhering to Company Policies and Procedures, state and federal guidelines, and other relevant regulations.
- Collaborate with the Interdisciplinary Team to assess resident needs and develop effective care plans.
- Coordinate and manage the daily Care Management meeting, ensuring the review of resident care and the setting of the Assessment Reference Date.
- Comply with federal and state regulations regarding the completion and coordination of the RAI process.
- Monitor MDS and care plan documentation for all residents, ensuring accurate and timely updates.
- Support the tracking system of MDS schedules, maintaining current resident information and accurate data entry.
- Complete accurate coding of the MDS, utilizing information obtained from medical record reviews, observations, and interviews with facility staff, residents, and family members.
- Attend interdisciplinary team meetings and other meetings to gather information, communicate changes, and maintain and update records.
- Continuously update knowledge base related to data entry and computer technology.
- Participate in quality assurance activities, as directed by the Resident Care Management Director.
- Complete the AANAC Certificate program within the first six months of employment.
- Communicate with the Medicare Beneficiary Hotline, as per company policy and Resident Care Management Director direction.
- Submit required documentation electronically to the State database and other entities, adhering to company policy.
Requirements:
- Registered Nurse with healthcare experience in a long-term care facility.
- Knowledge of the RAI process is required.
- Knowledge of Medicare is preferred.
