Post-Acute Liaison Job Description

Job Summary:

The Post-Acute Liaison is a key member of our team, responsible for generating business from referrals sources to meet the community's revenue and census goals. This role involves developing relationships with community hospitals, physician offices, and other community outreach programs to drive external sales and achieve the highest possible occupancy within budgetary and regulatory guidelines.

Key Responsibilities:

  • Develop and maintain relationships with external partners to drive referrals and business growth
  • Coordinate external professional relationships to support hospital throughput needs and safely coordinated care transitions
  • Engage with collaborative network leadership, risk holding physicians, and managed care partners to share insights and enhance program opportunities
  • Collaborate with admission coordinator and other key team members to ensure efficient referrals and care transitions
