Position Overview:

As a vital member of our healthcare team, the Registered Nurse Case Manager plays a crucial role in delivering nursing services aimed at enhancing the overall physical, mental, and social well-being of patients. Under the supervision of the Clinical Nurse Manager, you will monitor patient outcomes and collaborate with physicians and other healthcare professionals to ensure effective home health care plans.

Key Responsibilities:

  1. Conduct comprehensive assessments of patients' physical, emotional, socio-economic, and environmental needs.
  2. Deliver direct skilled nursing care as per physician orders, ensuring timely documentation of all care provided.
  3. Monitor and report changes in patients' physical and mental conditions to the physician, including when care goals are met or when care must be terminated.
  4. Provide counseling and education to patients, families, and caregivers to support their understanding and engagement in the care process.
  5. Participate in quality assurance initiatives and performance improvement activities, as well as agency-sponsored training sessions.
  6. Collaborate with patients, their representatives, caregivers, and physicians to develop, implement, and evaluate individualized care plans.
  7. After the initial 90 days, oversee and guide the clinical activities of LPNs, ensuring compliance with established protocols.
  8. Identify additional home health service needs for patients and initiate referrals as necessary, under the guidance of the Clinical Nurse Manager.
  9. Adhere to all compliance responsibilities associated with the role.
  10. Perform other related duties as assigned.

Qualifications:

Required: Strong research capabilities and excellent interpersonal communication skills are essential. Familiarity with systems integration is also necessary.

Minimum Experience: One year of relevant experience.

Desired: A Bachelor's Degree in Nursing and experience within a qualified home health agency.

Working Conditions:

Manual Skills: Significant manual dexterity and coordination required.

Occupational Risks: Some exposure to occupational hazards.

Physical Effort: The role demands very heavy physical effort, with the potential to exert up to 50 lbs. of force.

Physical Environment: May involve exposure to various environmental factors, including dirt, odors, noise, and human waste.
