Job OverviewThis role is responsible for coordinating patient care and navigating resources to optimize service quality and efficiency. The ideal candidate will possess strong communication and organizational skills, as well as the ability to work collaboratively with healthcare teams.
Responsibilities- Coordinates care for patients and families by developing personalized plans to assist with scheduling and addressing specific needs.
- Provides initial patient contact and establishes relationships to ensure seamless care coordination.
- Identifies patient trends and receives referrals from patients to coordinate appropriate housing and support services.
- Bachelor's Degree in Nursing; Master's Degree in Informatics or related field preferred.
- 1 year of experience in patient navigation or a related field.
- RN license required.
- Competitive salary: $85,000 - $110,000 per year, depending on experience.
- Sign-on bonus available.
