Job Description: The Social Services Coordinator position is a key role in our healthcare team. As a Social Services Coordinator, you will work closely with patients, families, and healthcare professionals to ensure the social needs of our patients are met.

Main Responsibilities:
  • Conduct interviews and assessments to identify social needs.
  • Develop and implement plans to meet social needs.
  • Coordinate care with healthcare professionals.
  • Provide emotional support and counseling to patients and families.
  • Assist with patient admissions and discharges.
  • Maintain accurate records and documentation.

Requirements: To be successful in this role, you must have a high school diploma and at least one year of experience in healthcare or long-term care. You should also be able to problem-solve, work well under pressure, and demonstrate neat appearance and good personal hygiene. A valid driver's license is also required.

Salary Range: $58,000 - $68,000 per year.
