Job Summary: We are seeking an experienced Social Services Coordinator Lead to join our team. In this role, you will be responsible for planning and implementing new ideas and programs that align with our facility's mission. You will also work closely with families, hospital discharge planners, social service agencies, and physicians to ensure seamless care coordination.

Responsibilities:
  • Demonstrate leadership skills by planning and implementing new ideas and programs.
  • Interview prospective residents and maintain records of inquiries.
  • Work as a liaison between families and healthcare professionals.
  • Assist with patient admissions and discharges.
  • Complete cover sheets for MDS forms.
  • Chair Admission Conferences/MDS meetings.
  • Attend required meetings and participate in Quality Assurance committee.

Requirements: To be successful in this role, you must have a high school diploma and at least one year of experience in healthcare or long-term care. You should also be able to problem-solve, work well under pressure, and demonstrate neat appearance and good personal hygiene. A valid driver's license is also required.

Estimated Salary: $55,000 - $65,000 per year.
