Nurse Case Manager/Private Duty Nurse/RN

We are seeking a highly skilled Registered Nurse to join our team as a Nurse Case Manager/Private Duty Nurse. This is a full-time, permanent position that requires a strong background in nursing and case management.
Key Responsibilities- Develop, monitor, evaluate, and revise member care plans to meet individual needs and optimize health care across the care continuum.
- Perform telephonic or face-to-face clinical assessments to identify, evaluate, coordinate, and manage member needs, including physical and behavioral health, social services, and long-term services.
- Identify members at high risk for complications and coordinate care with the member and healthcare team.
- Manage chronic illnesses, co-morbidities, and disabilities to ensure cost-effective and efficient utilization of health benefits.
- Conduct gap in care management for quality programs and assist with the implementation of member care plans by facilitating authorizations/referrals within benefits structure or extra-contractual arrangements.
- Registered Nurse (RN) license in Virginia or multi-state/compact license.
- Minimum 3 years of nursing experience, with a strong background in case management.
- Specialty certification within one year of eligibility (ACM, CCM, CCCTM, or RN-BC).
- 3 years of case management experience.
- 1 year of experience working directly with members who meet the conditions listed below: a. Members receiving private duty nursing services, b. Life-sustaining Ventilator dependent members, c. Vulnerable infant members, including those diagnosed with neo natal abstinence syndrome, classified substance-exposed, or admitted at NICU Level 3.
