Join the County of Los Angeles, CA team as a Community Health Worker/Lived Experience, playing a vital role in supporting parents and families in achieving their goals. As a Community Health Worker/Lived Experience, you will work closely with clients, providing guidance, coaching, and support to help them overcome challenges and achieve self-sufficiency.
Key Responsibilities:- Assist professional staff in providing support and referrals to clients.
- Provide guidance, coaching, and support to parents and children.
- Plan, coordinate, and lead group and one-on-one meetings with parents.
- Ensure all parents receive services from the Department to develop and achieve a community-based approach to increase reunification.
- Serve as a liaison between the Department and community agencies.
- Recruit and support parents as they engage them over the phone, in Regional offices, and Dependency Court.
- Provide recruitment outreach services and activities in effort to engage other potential parents.
- Serve as an advocate for client access to departmental and community resources.
- Facilitate relationships between the agency and the parents by communicating agency policies and programs to parents.
- Assist parents and family members in obtaining and completing application forms for benefits and services.
- Receive service referrals from social workers, eligibility workers, and medical case workers.
- Provide emergency services to parents by making referrals to appropriate supportive agencies.
- Assist parents in completing necessary forms to obtain needed services.
- Participate in departmental and community meetings and workgroups.
- Lived experience working with the public or with community groups performing duties such as interviewing clients concerning social service matters, answering questions, or providing information about health, mental health, and social services to clients.
- Had DCFS case closed for at least 12 months.
- Valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
- PHYSICAL CLASS II-Light: This class includes administrative and clerical positions requiring light physical effort that may include occasional light lifting to a 10-pound limit and some bending, stooping, or squatting.
- Experience coaching, mentoring, and training parents to achieve their parenting goals.
- Experience performing public relations work/professional maintenance of a favorable public image by a company or other organization, which must include interaction and collaboration with parents and/or community residents at community events.
- Experience in preparing comprehensive reports, completing projects, and analyzing reports using Microsoft Office.
