Responsibilities:
  • Develops and implements individualized plans of care for clients
  • Collaborates with treatment teams to coordinate services and secure sustainable housing
  • Provides advocacy services to ensure clients' needs are met
  • Documents progress and outcomes in a clear and concise manner

Requirements:
A bachelor's degree in a human service field or four years of equivalent education and/or experience working in the human services field is required. Computer literacy and a valid Kansas driver's license with access to personal vehicle are also necessary. Experience working in the community with individuals who have serious and persistent mental illness is preferred.

Estimated Salary:$55,000 - $65,000 per year
