Job Description

Capital Area Health Network is seeking a skilled Licensed Practical Nurse (LPN) to join our Wellness Center team. The Wellness Center is a comprehensive HIV and AIDS care facility that has been serving the Richmond community since 2001.

Key Responsibilities:

  • Triage patients with medical emergencies and coordinate follow-up care with medical providers.
  • Coordinate patient flow and ensure patient safety in the clinical setting.
  • Provide direct patient care, triage, and other duties as assigned.
  • Conduct health and educational assessments, and re-evaluations during intake.
  • Act as a patient advocate and consult with physicians as needed.
  • Assist with the coordination of resources with the interdisciplinary team.
  • Develop policies, procedures, and clinical protocols.
  • Participate in case conferences and provide supporting facts.
  • Maintain patient files in accordance with HIPAA.
  • Process routine, urgent, and stat referrals.
  • Understand insurance processes and requirements.

Requirements:

  • Valid Virginia driver's license and satisfactory driving record.
  • Licensed to practice in the state of Virginia, with a minimum of three years of nursing experience.
  • Current CPR (Healthcare Provider) certification.
  • Previous experience as a Licensed Practical Nurse in a community health and/or public health setting.
  • Leadership skills and critical thinking abilities.
  • Computer literacy and experience with computerized charting.
