Job Summary

We are seeking a skilled LPN/LVN to join our team at CenterWell Primary Care. As a Primary Care LPN/LVN, you will provide professional nursing care to patients within an assigned unit, clinic or region.

Key Responsibilities
  • Assess patient health problems and needs, take vital signs, administer medication, develop and implement nursing care plans, and maintain medical records.
  • Advise patients on health maintenance and disease prevention or provide case management.
  • Work within defined parameters to identify work expectations and quality standards, but have some latitude over prioritization/timing, and work under minimal direction.
Requirements
  • Bilingual
  • Phlebotomy experience
  • Experience with Electronic Medical Records
  • Experience with HEDIS
  • Knowledge of primary care medicine requirements (state specific)
  • Working knowledge of clinical operations
