Cortica is seeking a highly skilled Care Navigator to join its growing team. As a Care Navigator, you will play a critical role in guiding families through our range of services, ensuring a positive and patient-centered experience.
Key Responsibilities- Client Engagement: Ensure caregivers are aware of their welcome visit appointments, manage cancellations, and arrange substitute coverage as needed.
- Service Coordination: Schedule appointments based on welcome visit recommendations, consults, and support services within established service standards.
- Communication: Serve as the primary point of contact for warm transfers, answer questions, provide guidance, and review policies and next steps with caregivers.
- Client Support: Act as a liaison between patients and Cortica teams, ensuring a high level of client support and ease of navigating our ecosystem.
- Project Participation: Participate in projects to improve client experience, provide insights and feedback on enrollment and scheduling best practices.
- Experience: Two years of customer care experience, preferably in a healthcare setting.
- Skills: Excellent communication and interpersonal skills, ability to collaborate cross-functionally, and proficiency in software and systems, including electronic medical record systems and Microsoft Office products.
- Education: Bachelor's Degree in a Human Services or related field (preferred).
