Community Health Worker

We are seeking a highly motivated and compassionate Community Health Worker to join our team at David Raines Community Health Centers. As a Community Health Worker, you will play a vital role in supporting the health and well-being of our community members by providing education, guidance, and resources to help them achieve their health goals.
Key Responsibilities:- Provide health coaching and patient support to individuals and groups, focusing on healthy eating and exercise habits, and weight management.
- Recruit and enroll participants in our research study, the Collaborative Multi-level Obesity Intervention Engaging Underserved Communities Trial (CONNECT).
- Conduct surveys and measurements, and collect data to support the study's objectives.
- Engage in community outreach and facilitation of Community Asset Mapping to identify and promote resources that support healthy lifestyles.
- Work with community members, staff, and partners to build and maintain positive relationships and increase healthy behaviors through community projects.
- Associate's or Bachelor's degree in a related field.
- One year of directly related experience working with community programs in Louisiana.
- Research/data collection experience and public speaking/presentation experience.
- CHW training from a certified program (e.g., Louisiana Community Health Outreach Network).
