We are currently seeking full-time Direct Support Professionals (DSPs) to join our Day Habilitation Program. The Community Support Specialist plays a crucial role in assisting individuals with developmental disabilities to achieve their personal and vocational goals both in our center and within the community. This role also involves supporting the implementation and documentation of each individual's personalized program plan.

Role Summary

The Community Support Specialist will work directly with individuals with developmental disabilities, helping them to reach their full potential in both personal and vocational aspects. This position entails providing direct care support and assisting in the execution and documentation of each individual's program plan. Floating Community Support Specialists may be assigned to various programs based on staffing needs. The following standards apply to all Community Support Specialists.

Key Responsibilities

  1. Deliver and document services, supports, safeguards, and activities as outlined in the Individualized Service Plan (ISP) and Day Habilitation plan. Complete monthly notes to reflect the individual's response to services within the designated timeframe.
  2. Provide personal care as needed, including assistance with toileting, transferring, and hygiene, while documenting findings appropriately.
  3. Engage actively and communicate effectively with both staff and individuals served.
  4. Administer medications responsibly and maintain accurate documentation.
  5. Build a trusting and friendly rapport with individuals while upholding professional boundaries.
  6. Act as a mandated reporter by promptly reporting any suspected abuse to management.
  7. Maintain a safe, clean, and welcoming environment within the center.
  8. Demonstrate proficiency in the NYS DSP Core Competencies, which will be the basis for performance evaluations.
  9. Provide transportation for individuals using agency vehicles, accommodating up to 14 passengers.
  10. Ensure all required trainings and certifications are up to date and seek additional training as necessary.
  11. Exhibit professionalism, respect, and teamwork in all interactions.
  12. Support the agency's vision and mission.
  13. Adhere to the agency's Code of Legal and Ethical Behavior, ensuring responsibilities are performed with integrity and respect.
  14. Follow all agency policies and procedures, prioritizing personal and others' safety.
  15. Document observations regarding individuals' progress, behavior, and any relevant incidents.
  16. Perform other job-related tasks as assigned by supervisors.

Qualifications

  • High School Diploma or equivalent is required. Experience with individuals with developmental disabilities is preferred.
  • Basic computer skills are necessary.
  • A valid NYS driver's license is required, subject to insurance approval.
  • Current certifications in SCIP R, Medication Administration, CPR, and SFA, or the ability to obtain them.
