About the RoleAs a Home Health Physical Therapy Assistant, you will play a vital role in providing high-quality care to our clients in the comfort of their own homes. You will work closely with our licensed Physical Therapists to develop and implement individualized care plans, providing manual therapeutic exercises, gait training, balance, and other interventions to help our clients achieve their goals.
Responsibilities- Assist clients in attaining or maintaining optimal physical wellbeing by providing treatment, tests, client education, and appropriate interventions as delegated by a licensed Physical Therapist.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Educate and instruct clients, family members, or other client representatives in rehabilitative care and activities necessary to promote the health, safety, and independent living of clients.
- Accurately document observations, interventions, and evaluations pertaining to client care management.
To be successful in this role, you will need:
- A current Physical Therapist Assistant license in the state of Hawaii.
- Graduation from a two-year college-level program approved by the American Physical Therapy Association, or two years of appropriate experience as a Physical Therapist Assistant and a satisfactory grade on a proficiency exam conducted, approved, or sponsored by the U.S. Public Health Service when initial qualification occurred before December 31, 1977.
- A minimum of one year of recent work experience under the supervision of a qualified Physical Therapist.
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
