As a Staff Nurse LPN, you will be responsible for administering medications, completing treatments, and providing education to residents and their families. You will also participate in developing and implementing individualized care plans, collaborating with multidisciplinary teams, and maintaining accurate records of resident care. Key Responsibilities:
- Deliver high-quality patient care, adhering to established standards and protocols.
- Collaborate with multidisciplinary teams to ensure seamless care delivery and make recommendations for improvements.
- Develop and implement individualized care plans, taking into account resident medical history, preferences, and goals.
- Provide education to residents and their families on various aspects of healthcare, including disease management, wound care, and medication adherence.
Requirements:
- Graduation from formal LPN training program as a Licensed Practical Nurse.
- One year of LPN experience in a SNF convalescent or nursing home, hospital, or related healthcare facility.
- Must possess Connecticut LPN License at time of application.
- Knowledge of geriatric nursing and pharmacology.
- Skilled in clinical competency and effective communication abilities.
Salary:
Competitive salary range of $34.46 - $38.29 hourly.
