We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team at Home Care Connectors. As a key member of our healthcare team, you will be responsible for delivering high-quality nursing care to patients in their homes.
About the JobIn this role, you will assess patient needs, develop individualized care plans, and provide compassionate care, including medication administration, wound care, and education on health management.
Essential Responsibilities- Assess patient needs and create personalized care plans
- Provide expert nursing care, promoting patient well-being
- Foster effective communication with patients, families, and healthcare teams
- Ensure accurate record-keeping and reporting
- Current LPN licensure in Connecticut
- Minimum one year of nursing experience
- Exceptional interpersonal and communication skills
This position offers a competitive salary package, including:
- $70,000 - $75,000 per year, based on experience
