Conifer Health Solutions is seeking a skilled Patient Advocate Representative to join our team. As a key member of our healthcare team, you will play a vital role in ensuring that patients receive the best possible care and support.
Key Responsibilities- Conduct thorough interviews with patients and/or family members to assess their needs and eligibility for various programs.
- Record and maintain accurate documentation of all activities performed on patient accounts, including financial clearance and collections.
- Develop and maintain strong relationships with patients, hospital staff, and community resources to ensure seamless care coordination.
- Collaborate with hospital case management, social services, and admissions staff to ensure timely and effective communication.
- Conduct field visits to patient homes for skip tracing and document collection.
- High school diploma or equivalent required; minimum 2 years of experience in social services or hospital admitting.
- Working knowledge of Federal, State, and County programs, as well as proficiency in Microsoft Office and Windows.
- Excellent communication and interpersonal skills, with the ability to work independently and prioritize multiple tasks.
- Bilingual (Spanish) preferred.
This role requires the ability to work in a fast-paced hospital environment, with frequent interaction with patients, staff, and community resources. Some travel may be required.
