Job Title: Medical Home Health Nurse

VNA Health Group is seeking a skilled and compassionate Medical Home Health Nurse to join our team. As a vital community resource for over 120 years, we provide innovative and coordinated care to individuals and families in their homes and communities.

The ideal candidate will have experience in home health care and a strong understanding of the nursing process. They will be responsible for providing direct or indirect comprehensive nursing care to patients, utilizing principles of prevention, therapeutic, and rehabilitative nursing care.

Key Responsibilities:

  • Provide skilled nursing care to patients in their homes
  • Assess patient needs and develop individualized care plans
  • Collaborate with healthcare teams to ensure seamless care coordination
  • Communicate effectively with patients, families, and healthcare providers

Requirements:

  • ~1-2 years' experience in Med Surg or hospital setting, preferred
  • ~ Completion of an accredited RN education program with a diploma, Associate's or Bachelor's degree
  • ~ Experience in home health care and 1 year experience in medical-surgical nursing
