Job Title: Per Diem RN Home Health Registered Nurse

VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our Home Health team. As a Per Diem RN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver exceptional outcomes.

About the Role:
  • Provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
  • Collaborate with patients, families, and healthcare providers to develop and implement individualized care plans.
  • Utilize evidence-based practice and clinical expertise to deliver high-quality patient care.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Participate in ongoing education and professional development to stay current with best practices and industry developments.
Requirements:
  • Current RN licensure in the state of Ohio.
  • 1-2 years of experience in medical-surgical nursing or home health care.
  • Completion of an accredited RN education program with a diploma, Associate's or Bachelor's degree.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
