Job Title: Registered Nurse - Medical Home

VNA Health Group is seeking a skilled Registered Nurse to join our team in Brooklyn Heights, Ohio. As a per diem / prn nurse, you will provide high-quality care to patients in their homes, working closely with our interdisciplinary team to deliver comprehensive care.

Job Summary:

We are looking for a compassionate and dedicated Registered Nurse to provide medical home health care to patients in need. As a member of our team, you will be responsible for assessing patients' needs, developing individualized care plans, and providing direct patient care. If you are a motivated and detail-oriented nurse with excellent communication skills, we encourage you to apply.

Responsibilities:
  • Assess patients' physical and emotional needs, developing individualized care plans to meet their unique requirements.
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
  • Document patient progress, including medical history, treatment plans, and outcomes.
Requirements:
  • Current RN licensure in the state of Ohio.
  • Minimum of 2 years of experience in home health care or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
