Position Overview

NUTRITION SPECIALIST

POSITION REQUIREMENTS:

  1. Must hold a valid Registered Dietitian credential.
  2. A bachelor's or higher degree from an accredited institution in nutrition or dietetics is essential.
  3. Minimum of one year of experience working with elderly or frail individuals is required.

SUMMARY: Appalachian Agency For Senior Citizens Inc is dedicated to providing comprehensive, community-based healthcare for the elderly population through its Program of All-Inclusive Care for the Elderly (PACE). The Nutrition Specialist plays a crucial role as part of the Interdisciplinary Team (IDT), delivering essential nutrition services to participants enrolled in the PACE program.

KEY RESPONSIBILITIES:

  1. Adhere to all State and Federal regulations regarding patient care, service delivery, participant rights, and Quality Improvement (QI) activities.
  2. Complete a criminal background check as mandated by the agency.
  3. Commit to the philosophy, practices, and protocols of the PACE organization.
  4. Engage in PACE Orientation sessions.
  5. Provide only those services authorized by the PACE IDT.
  6. Complete necessary competency evaluations for all staff involved in direct participant care.
  7. Conduct initial and ongoing assessments for PACE participants.
  8. Contribute to the development of the IDT Plan of Care and attend IDT meetings as required.
  9. Communicate any changes in participant status to team members.
  10. Assist in coordinating 24-hour care delivery.
  11. Ensure all documentation in the medical record is completed as required.
  12. Offer nutrition counseling and education to participants and caregivers as needed, adhering to the standards set by the Academy of Nutrition and Dietetics (AND).
  13. Provide nutrition education to staff and the broader community.
  14. Deliver nutrition education at Congregate Nutrition Sites at least quarterly.
  15. Offer nutrition counseling to eligible home-delivered meal clients, case management clients, and nutrition site members.
  16. Collaborate with team members to implement participant education initiatives.
  17. Procure nutrition supplements and snacks for Adult Day Care (ADC) in accordance with established protocols.
  18. Develop monthly snack menus for the ADC.
  19. Review the master agency lunch menu to ensure compliance across all programs.
  20. Coordinate with the agency's Nutrition Services Department regarding Home Delivered Meals and ADC meals.
  21. Assist the Center Manager in overseeing the PACE kitchen to ensure regulatory compliance.

KNOWLEDGE, SKILLS, AND ABILITIES:

  1. Ability to apply nutrition therapy principles effectively to the senior demographic.
  2. Skill in defining problems, setting goals, and developing interventions for care plans.
  3. Understanding of Quality Assurance principles and their application in various scenarios.

  9. Proficiency in data management and word processing software.
