Job Overview

We are seeking an experienced Care Coordination Professional to join our team at the Minnesota Assistance Council for Veterans (MACV). As a Care Coordination Professional, you will play a vital role in ensuring that Veterans receive comprehensive care and support to maintain housing stability.

About the Role
  • You will provide non-clinical, bio-psycho-social assessments of Veterans in collaboration with interdisciplinary treatment teams.
  • Through comprehensive care coordination, you will support Veterans in communicating their preferences and personal health-related goals to facilitate shared decision-making.
  • You will collaborate with community agencies and resources to maximize the use of VA and community resources.
  • As a key member of our team, you will contribute to the MACV mission to end Veteran homelessness in Minnesota.
Requirements
  • Masters-level social worker or BSW with 3+ years of experience, or equivalent.
  • Strong communication and interpersonal skills, with the ability to interact effectively with Veterans, colleagues, and community partners.
  • Ability to work independently, exercising initiative and judgment in day-to-day activities.
  • Knowledge of community health care benefits and services, including Medical Assistance, Minnesota Care, and Medicaid.
Salary and Benefits

The estimated salary for this position is $$58,000 - $68,000 per year, depending on experience. We offer a competitive benefits package, including healthcare coverage, dental, vision, life insurance, paid time off, retirement benefits, and more.
