Job Summary

We are seeking a skilled Veterans Healthcare Access Coordinator to join our team at the Minnesota Assistance Council for Veterans (MACV). This is a critical role in supporting our mission to end Veteran homelessness in Minnesota.

About the Role

This position will focus on connecting Veterans with VA health care benefits or community health care services, providing case management and care coordination, and facilitating access to resources that support housing stability.

Key Responsibilities
  • Provide non-clinical assessments of Veterans' needs and develop personalized care plans.
  • Connect Veterans with community health care services, including behavioral health and substance use treatment providers.
  • Coordinate with partner agencies and staff across all MACV programs to ensure comprehensive care.
  • Assist Veterans who are homeless or at risk of homelessness in accessing health services that promote housing stability.
  • Conduct regular reviews of care plans and make adjustments as needed.
Requirements

To be successful in this role, you should have:

  • A minimum of two years of experience in a healthcare or social services area of practice.
  • Excellent judgment and communication skills.
  • The ability to work independently and exercise initiative and judgment in day-to-day activities.
  • Strong problem-solving and analytical skills.
  • A deep understanding of the challenges faced by Veterans and their families.
