Job Description:

As a Senior Clinical Care Coordinator - Nephrology, you will be responsible for providing exceptional patient care in an outpatient dialysis setting. Your primary focus will be on building strong relationships with patients and their families, while collaborating with healthcare professionals to deliver high-quality services.

Key Responsibilities:

  • Patient Care: Provide direct patient care, including administering medications, conducting vital sign measurements, and managing patient equipment.
  • Care Coordination: Coordinate patient care activities, ensuring seamless transitions between treatment sessions and hospital admissions.
  • Team Collaboration: Work closely with multidisciplinary teams, including doctors, nurses, and technicians, to develop and implement comprehensive care plans.

Requirements:

  • Education: Associate Degree in Nursing (A.D.N) from an accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from an accredited diploma program may be substituted for nursing degree.
  • Experience: Intensive Care Unit (ICU), Critical Care Unit (CCU), Emergency Room (ER), or Medical Surgical (Med Surg) experience preferred but not required.
  • Certifications: Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) certification highly valued.
