Job Summary:

Penn State Health is seeking a dedicated Registered Nurse to join our team as a Diabetes Care and Education Specialist. In this role, you will have the opportunity to make a meaningful impact on the lives of patients and families affected by pediatric diabetes.

About the Role:

As a Diabetes Care and Education Specialist, you will be responsible for providing high-quality patient care, education, and support to patients with diabetes and their families. You will work closely with healthcare teams to develop and implement individualized care plans that promote optimal health outcomes.

Responsibilities:
  • Provide comprehensive patient care, including medication management, wound care, and glucose monitoring
  • Develop and deliver educational programs for patients, families, and healthcare professionals on diabetes self-management and prevention
  • Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions between settings
  • Conduct assessments and evaluations to identify patients at risk for complications and develop targeted interventions
Requirements:
  • Bachelor's degree in nursing required; Master's degree preferred
  • Current licensure as a registered nurse in Pennsylvania
  • At least one year of experience in pediatrics or a related field
  • Strong communication and interpersonal skills
