- Document safety events and assist in resolution under the guidance of the Safety Officer.
- Provide patient education on medications and disease states, and facilitate medication access by assisting with prior authorizations and other Patient Care Coordinator activities.
- PharmD degree from an accredited college of pharmacy.
- At least 2 years of experience managing patients' specialty medication needs or clinical pharmacist hospital practice experience.
- Excellent verbal and written communication skills.
- Proficiency with Microsoft Office.
- Current State pharmacy license.
