Key Responsibilities
  • Provide medication management through comprehensive evaluations of patients' medication regimens to ensure safe and effective utilization.
  • Collaborate with patients, providers, nurses, and other healthcare professionals to ensure optimal patient outcomes.
  • Participate in clinical meetings and training sessions to maintain proficiency in pharmacy practice and stay up-to-date on the latest developments in the field.
  • Document safety events and assist in resolution under the guidance of the Safety Officer.
  • Provide patient education on medications and disease states, and facilitate medication access by assisting with prior authorizations and other Patient Care Coordinator activities.
Requirements
  • PharmD degree from an accredited college of pharmacy.
  • At least 2 years of experience managing patients' specialty medication needs or clinical pharmacist hospital practice experience.
  • Excellent verbal and written communication skills.
  • Proficiency with Microsoft Office.
  • Current State pharmacy license.
