Job Responsibilities
  • Deliver Post-Acute Care: Provide high-quality post-acute care as an LPN, monitoring the physical and mental wellbeing of residents.
  • Document Resident Information: Perform intakes of residents, documenting their health concerns and medical histories.
  • Administer Medications: Properly administer and verify medications, ensuring the highest level of care.
  • Maintain a Safe Environment: Contribute to a safe, comfortable environment for residents, assisting with IV set up, vaccinations, and phlebotomies.
Qualifications and Skills
  • Current Illinois State License: Hold a current Illinois state license to practice as a Licensed Practical Nurse (LPN).
  • LPN Educational Program: Complete an approved Licensed Practical Nurse (LPN) educational program.
  • High School Diploma: Possess a high school diploma or equivalent.
  • Minimum Experience: Have a minimum of 6 months of experience as an LPN or in a related role.
