Job Title: Licensed Practical Nurse (LPN)Job Summary:

IntelyCare is seeking a skilled Licensed Practical Nurse (LPN) to join our team. As an LPN, you will have the opportunity to work in a variety of settings, including post-acute care, and provide high-quality patient care. Responsibilities:
  • Deliver post-acute care to patients, including monitoring vital signs and administering medications
  • Conduct intakes and document patient health concerns and medical histories
  • Review medical records and charts to ensure accurate patient care
  • Administer medications and verify dosages
  • Maintain a safe and comfortable environment for patients
  • Assist with IV set up, vaccinations, and phlebotomies
  • Follow and adhere to care plans for patients
  • Assist with medical staff orientation and training
Requirements:
  • Current Illinois state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of approved Licensed Practical Nurse (LPN) educational program
  • High school diploma (or GED/equivalent)
  • Minimum of 6 months of experience (LPN or related role)
  • Ability to lift a minimum of 50 lbs
