Job SummaryWe're seeking an experienced MDS Coordinator RN to join our team at Trilogy Management Services, LLC. As a key member of our care team, you'll be responsible for assessing residents' needs and creating personalized care plans.
Key Responsibilities- Manage the Resident Assessment Instrument (RAI) process to assess residents' individual needs and create unique care plans.
- Promote cross-functional communication to ensure care plans are followed and residents receive high-quality care.
- Ensure accurate reimbursement for resident care services by staying up-to-date on federal, state, and local standards, guidelines, and regulations.
- Current, valid state RN license
- 2+ years of MDS (resident assessment) experience
- Prior experience in Long-Term Care is required
